Conveniente…Necesario…Indispensable, son tres nociones que se apegan al devenir histórico y social de esta obra. Hace dos décadas, la primera edición de Medicina en Odontología. Manejo dental de pacientes con enfermedades sistémicas, abordaba lo “conveniente”. Era una época en la que el objetivo fundamental tanto de la Medicina como de la Odontología se enfocaba en la atención al daño más que a las enfermedades mismas, marcando conceptualmente a la enfermedad como: “de Facto”. Consecuentemente en aquel entonces, el contenido y orientación del texto aportaba herramientas preventivas a los Odontólogos para evitar que el daño médico establecido se agravase debido al tratamiento bucal.
En sólo pocos años cambian los valores; el concepto de salud y su preservación preponderan contraviniendo al de enfermedad. Los esfuerzos en diversas esferas locales y globales, se dirigen entonces a buscar la conservación de ese estado, o bien, en su ausencia, a una pronta y eficaz recuperación. En beneficio de los pacientes, en ocasiones ante la imposibilidad de recuperar la salud se propicia el control de la enfermedad. El avance científico y cultural logra que muchas de las enfermedades que aquejan al serhumano se mantengan en un estado crónico conveniente, que permite a los pacientes alcanzar más años de sobrevivencia en condiciones cercanas a las de una vida normal.
De ahí surge el concepto de “necesario”, que aplicamos a este texto, ya que en los consultorios odontológicos es cada vez más frecuente la presencia de pacientes solicitantes de servicios estomatológicos que presentan enfermedades sistémicas crónico-degenerativas bajo control, de etiología y evolución diversa bajo tratamiento médico de diferentes tipos, individuos que en épocas pretéritas muy recientes, , no formaban parte de su “mercado”.
En Medicina y Odontología preventivas también germinan transformaciones y en los foros globales aparece la intención de la conservación y promoción del estado de salud, bajo la premisa de que biológica, social y económicamente se convierte en una mejor opción. Concepto como nivel de riesgo y empoderamiento surgen en los medios médicos, gubernamentales, en las asociaciones gremiales y agencias internacionales. Aparecen también las primeras nociones de la interrelación e interdependencia de “lo local y lo sistémico”, manifestándose entonces el concepto en ciernes de Medicina Periodontal.
